Donna Lee Burnham, 74, of Columbia, MO, formerly of Bloomfield and Ottumwa, IA, passed away peacefully on August 14, 2015 of kidney failure.
A memorial service will be held in Donna’s hometown of Bloomfield, IA at Lake Fisher Park’s large shelter on Saturday, October 10, 2015 at 12:30 p.m. A reception will follow at Bloomfield Christian Church 1-4 p.m. In case of inclement weather, the memorial service will also be held at Bloomfield Christian Church.
Donna was born December 31, 1940 in Floris, Iowa to Francis Bernard and Margaret Alice Lamb Smith and they preceded her in death. Donna graduated from Davis County Community High School in Bloomfield in 1959. On September 11, 1960 she married Roy Dean Forgy in Bloomfield, Iowa. On October 26, 1978 she married W. Dean Burnham in Lancaster, Missouri.
She is survived by two sons, Clint Forgy of Columbia, MO and Kirk Forgy (Carol) of Fulton, MO; a daughter, Jennifer Burnham (Jeff) Clouse of Columbia, MO; three brothers, Frank (Dee) Smith and Dan (Lyn) Smith, both of Bloomfield, IA; Jack (Pat) Smith of Modesto, CA; and a sister, Linda (Art) Welch of Millersburg, MO; six grandchildren, Derik (Missy) Forgy, Zackery Forgy, Joshua Forgy, Sydney Harris, Jessica Forgy, and Nickolas Forgy, and four great grandchildren, Jordan, Harper, Genevieve and Rylan.
